The NBA has noticed that many of the branches have challenges in availing the National Secretariat the necessary documentations to process the death benefits of members timeously. This documentation lapse usually causes delays in processing the payment to the beneficiaries.

The National Welfare Secretary, Kunle Edun is presently on top of all the pending issues with a view to ensuring that the process is understood by all regarding proper documentations and payment of claims. All branches are hereby required to co-operate in this regard.

Please, Note that the required documents must be scanned together in PDF format. The documents should be emailed to Mr. David Ezekiel at david.ezekiel@nigerianbar.org.ng and copy eduovo@yahoo.com

Upon receipt of the complete documents, the Leadway Assurance Death claim forms would be availed the applicant and expected to be filled by the beneficiary and returned.

The required documents are:

* Valid means of identification of the beneficiary e.g. NIMC, VOTER’S CARD

* Valid means of identification of the deceased e.g NIMC, VOTER’S CARD

* Death certificate

* Medical report

* Evidence of payment of BPF and local dues for the year the deceased member passed on

All applications must state the SCN of the deceased member.

Should any member require further information or clarification, pls kindly send an email or call *Kunle Edun on 08038695936* or Ezekiel on 07037000903. For the purpose of processing the insurance claims of deceased members, the communication shall be between the NBA National Secretariat and the Chairmen/Secretaries of the branches of deceased members.

Your welfare is our priority.
