The Kwara State Judiciary has released a checklist for Affidavit/Declaration Of Age/Marriage.

In a Notice signed by the Deputy Chief Registrar (Litigation) and sighted by TheNigeriaLawyer (TNL), any form of extortion are enjoined to be reported.

CHECKLIST FOR A VALID AFFIDAVIT/DECLARATION OF AGE/MARRIAGE.

*THE DECLARANT/DEPONENT MUST BE PRESENT.

*THE DECLARANT/DEPONENT MUST HAVE A VALID MEANS OF IDENTIFICATION I.E VOTERS CARD, INTERNATIONAL PASSPORT, DRIVERS LICENSE, NATIONAL IDENTITY CARD OR BVN PRINT OUT.

*SAVE FOR DECLARATION OF AGE AND DECLARATION OF MARRIAGE, ALL AFFIDAVITS MUST HAVE AT LEAST TWO (2) PASSPORT PHOTOGRAPHS.

*AFFIDAVIT ON CHANGE OF OWNERSHIP SHOULD BE ACCOMPANIED BY AN AGREEMENT FRANKED BY A LEGAL PRACTITIONER.

*FOR DECLARATION OF MARRIAGE, ONE OF THE REPRESENTATIVES QUOTED ON THE FORM MUST BE PRESENT WITH THE DECLARANT.

*ANY AFFIDAVIT AUTHORIZING THE DEPONENT TO DO AN ACT ON BEHALF OF OTHERS MUST BE ACCOMPANIED WITH THE INSTRUMENT OF AUTHORITY DULY SIGNED BY THE AUTHORIZING BODY OR PERSON(S).

*ANY AFFIDAVIT ON CHANGE OF NAME SHOULD STATE COGENT REASONS FOR THE CHANGE.

*ORDERLINESS AND DECORUM MUST BE MAINTAINED WITHIN THE COURT PREMISES.

*ANY FORM OF EXTORTION SHOULD BE REPORTED TO THE AUTHORITY.
